Adequate medical records are important to the well being of our entire camp
community. No camper will be received in camp without a current Health
Information Form. Please attach a photocopy of your
child’s Ontario Health Card, if applicable.
IMPORTANT: In order to aid us in the proper care of your child, please
advise us of any special conditions of health, diet, or personal habits that
affect your child. This information should be in writing and attached to
the Health Information Form or sent in a personal communication to the camp
director. Also, should the state of your child's health change or should
s/he be exposed to any communicable disease during the 3 weeks prior to
camp, the camp office must be informed.
BOOSTERS, INJECTIONS, AND INOCULATIONS
Has your child been fully vaccinated against polio, tetanus and other
diseases? Please consult your physician. Most recent tetanus immunization
must be specified on the Health Information Form. It is advisable to
complete all such inoculations by the end of May.
INSURANCE
The camp accident insurance automatically covers the camper in the event of
an accident according to the terms of our policy. In the case of sickness,
we will look to your own health insurance coverage for primary payment of
bills incurred. In order to prevent mishandling of insurance matters, please
make certain that the name of your health insurance company and policy
number appear on the camper Health Information Form as requested.
SPECIAL MEDICATIONS
Should your child require special medications, please complete the
Medication Form. One copy must be sent to our office by May 15, 2008;
another copy is to be included with the medication and handed to a camp
staff member at the point of bus departure.
MEDICAL CONSULTATIONS
Should special medical or surgical procedures be required, the camp will
make every effort to reach and consult with parents in advance. The camp
will also automatically inform the parents in case of any dispensing of
antibiotic or confinement of a camper to the infirmary.
EYEGLASSES
Children who wear glasses should bring an extra pair to camp in case of
breakage or loss. It would also be a good idea to bring a copy of their
latest prescription to be left on file in the infirmary. Please make sure
your child’s name is marked on their eyeglasses.
DENTAL WORK
It is important that children’s dental needs be attended to before camp.
There is no dentist in camp, and it is difficult for us and expensive for
you to have us take children to a dentist in town. In the event that your
child must have dental or orthodontic treatment, you will be billed for this
service. Camp medical insurance does not cover dental work.
HAIRCUTS
Every camper should have a haircut before departing for camp. There will be
no such services provided at camp.
Please have your child’s hair checked for head lice within a week prior to
camp.
